How to Request Your Health Care Records

Clients may request copies of their health care records maintained by White Willow Psychological Services by submitting a written request.

To request records:
Submit a written request (email or secure message preferred)
Include your full name, date of birth, and specific records requested
Requests must be signed by the client or the client’s legal representative

Contacting the Licensing Authority: Texas Behavioral Health Executive Council

Consumers may contact the Texas Behavioral Health Executive Council (TBHEC) regarding any licensed provider at White Willow Psychological Services, including providers licensed as:

• Psychologists

• Social Workers

• Professional Counselors

• Marriage and Family Therapists

The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology.
Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.
